There has been a growing recognition of the need to develop inter-professional models of care for patients with chronic and/or complex conditions. A case conference is a meeting of health professionals to plan care for an individual patient. Community pharmacists in Australia have been recognised as key members of the health care workforce.. The access to the whole abstract and the presentation file is available to FIP members and to congress participants of that specific congress.
