Background: Demand for pharmacist practice change is strong. Patient-centred pharmaceutical care delivery to prevent and resolve medication-related problems is required. Collaborative medicines reviews (CMR) are one example of this practice. Practice change however is slow. We report on a novel approach to drive practice change by increasing..
