  Background information: The medical team cooperate with each other to improve treatment is well known. Most of the member as a team including physician, nurse, and dietician. Is there need of pharmacist joining the multidispline-team to give information for pharmacotherapy or evaluate the possibility of drug-related problem is still intreasting.
