Background:
Dispensing errors due to look-alike medication names, labeling and packaging are common in the hospital pharmacy. Pharmacist plays a very important role in managing risks associated with dispensing. In this study, a survey was conducted to analyze the reasons for the errors and evaluate whether or not changing medication labeling on..
