BackgroundMedication reconciliation (MedRec) is a patient-centered evolving role of pharmacists.
MethodsA prospective pre-post intervention study was carried out at University College Hospital (UCH), Ibadan (intervention site) and University of Ilorin Teaching Hospital (UITH), (control site). Pharmacists’ knowledge on MedRec was assessed.. The access to the whole abstract and the presentation file is available to FIP members and to congress participants of that specific congress.
