    Background

    The medication reconciliation process is hampered by the complexity, heterogeneity, and lack of a standardised method for quantifying the outcomes.

    Methods

    The taxonomy of medication discrepancies was developed and tested for content validity, inter-rater reliability, internal consistency, and usability between pharmacists and nurses.
