Introduction: Australia’s ageing population, decreasing healthcare workforce and the marked increase in chronic disease has led Government healthcare policy to promote collaborative chronic disease management centred on the general practitioner (GP). Community pharmacists (CPs) have the potential to improve collaborative chronic disease..
