    Background

    In 2017, those above 65 years-old accounts for more than 13.7% of the Taiwanese population. Among the elderly, 81.1% receive more than five prescriptions. Consequently, increased adverse drug events. Therefore, Shin Kong Wu Ho-Su Memorial Hospital’s Department of Pharmacy has routinely assigned pharmacists to long-term care (LTC)..
