    Background

    Polypharmacy is increasing in a growing elderly population. One way to assess the medication is to perform medication reviews that identify drug-related problems (DRP). Medication reviews of elderly patients’ medication can affect many parameters, e.g. adherence and discontinuation of potentially inappropriate medication.
