Background
For long, adverse drug reaction (ADR)-reporting by pharmacists in our hospital is relatively low, the reporting rate per month is less than 5% of all ADRs.Methods
T-codes, belongs to ICD-10-CM codes, would indicate the drug class responsible for the adverse drug event (e.g., T36, poisoning by, adverse effect of and underdosing of systemic .. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
