  • Abstract:

    Background

    Adverse Drug Reaction (ADR) is the side effect or unexpected reaction after patients taking legal drugs under pharmacist’s instruction. To report and document ADR can help pharmacists to realize patient history of drug allergy and also avoid recurrence of the same ADR, which may endanger patient’s safety.
