In hospital pharmacies, mistakes are expected. Mistakes usually result from distraction. Voluntary reporting could reduce future error.
Data in this cross–sectional study was part of data collected to assess patient safety culture among hospital pharmacists in Alexandria using original validated questionnaire “Pharmacy survey on Patient safety..
