Statistically, elderly being admitted for adverse drug reactions is four times higher than the young, and out of 88% of the incidents are preventable. Thus, Shin Kong Wu Ho-Su Memorial Hospital, designate pharmacists to long-term care facilities for drug regimen review (DRR), study the drug-related problems (DRPs) and the potentially inappropriate ..
