Pharmacists in Japan now play a key role in a patient’s care in hospital. Their responsibilities include filling prescriptions, checking a patient’s medication history and providing appropriate information to other health-care workers. More importantly, pharmacists’ interventions can also result in the reduction of adverse drug reactions (ADRs).. The access to the whole abstract and the presentation file is available to FIP members and to congress participants of that specific congress.
