  ADAPT, an online CE program launched in 2010 by the Canadian Pharmacists Association, uses a cognitive apprenticeship model to teach medication and patient assessment, collaboration, evidence based decision making, documentation and care plan preparation to enable pharmacists to embrace expanded scope and medication management services.
