An unacceptable gap exists between evidence-based guidelines for anticoagulation therapy and implementation of such guidelines in actual clinical practice. One such example pertains to systematic models of anticoagulation care such as anticoagulation management services. This presentation will 1) review examples of anticoagulation medication..
