BACKGROUND- No hospital level survey to comprehensively assess hospital pharmacy practice exists. AIMS- To pilot and validate a survey tool for international hospitals to use in assessing their levels of pharmacy practice as described in the Basel Statements. METHODS - We reviewed the Basel statements to identify characteristics that could be..
