    Background:
    Field survey in Tainan City found that polypharmacy and doctor shopping were common among local high-risk groups of drug-related problems. Therefore, Tainan City Government and 2 local Pharmacists Associations collaborated the community pharmaceutical care plan on the high-risk groups.
