  I. Background :
  Near miss is one of the important factors in making “Medication errors” by reporting from nursing station.
  
  
  II. Aims:
  To assure the quality of “Pharmaceutical Care ” and verify the correction of the prescription is the best way to prevent “medication problem” from its happening.
  
  
  III. Method :
  Collected 73 cases within two years from 2010
