    Background: Prescribing errors are known to be an important cause of morbidity and mortality in primary care and yet there have been few large-scale studies investigating the prevalence of these errors. Aim: To determine the prevalence and nature of prescribing and monitoring errors in general practices. Setting: Fifteen English general practices.
