  AIM: To show that the presence of a pharmacist on a hospital ward could improve medication safety._
  METHOD: To examine reported undesired events and compare them to observed events.
  To carry out a risk analysis based on the observed events and to use this analysis for prioritizing interventions.
  FINDINGS: Medication errors were underreported.
