Background: Medication errors (MEs) commonly occur at interface of care. MEs on hospital discharge are high risk for patients and may contribute to adverse drug events (ADEs) at home. The objectives of this study were to determine MEs and DRPs by pharmacist.
Setting: At general medical wards, Ramathibodi Hospital, a teaching care hospital.. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
