    Background: To reduce the risk of medication errors with high-alert drugs in paediatric patients, the Canadian Council on Health Services Accreditation, the Joint Commission for Accreditation of Healthcare Organizations and the Institute for Safe Medication Practices endorses the standardization and limiting of drug concentrations available..
