Medication reconciliation (MedRec) is a patient-centered evolving role of pharmacists.
A prospective pre-post intervention study was carried out at University College Hospital (UCH), Ibadan (intervention site) and University of Ilorin Teaching Hospital (UITH), (control site). Pharmacists’ knowledge on MedRec was assessed..
