    Background

    Patients with long-term conditions (LTCs) have a role in managing their health and wellbeing.[1] The Greater Manchester Community Pharmacy Care Plan (GMCPCP) pilot was designed to support patients to set health goals, achieve better LTC management and improve quality of life using tailored community pharmacy care plans.
