Hospital inpatients have different levels of clinical acuity, some requiring greater intensity of care input compared to others. In the UK, most patients will be seen by a pharmacist daily, regardless of need. An existing Pharmaceutical Assessment and Screening Tool (PAST)1 was adapted for use by junior pharmacists to assign patient..
