Background
Polypharmacy induces drug-related problem and medication nonadherence in elderly, which increasing load for patients and also healthcare systems.Methods
Outpatients (≥ 65 years old) with polypharmacy (≥ 5 chronic drugs) were recruited from clinic and drug-counseling center and we surveyed their chart profile from the hospital information..
