It has been estimated that 30% of all hospital discharges are delayed for non-medical reasons. Identifying certain patients at higher risk for low medication literacy and delays at discharge could ultimately help improve the discharge process. Re-engineering Discharge Services and including pharmacists in the process has the potential to improve..
